Residents across Bath and North East Somerset, Swindon, and Wiltshire are set to benefit from a major overhaul of health and social care services. HCRG Care Group, a new provider appointed to lead the charge, will introduce a more integrated and streamlined approach to community-based care, working in collaboration with the NHS, local authorities, and various charities.
This new care model will bring vital health and social care services closer to home, with traditional services like nursing, therapy, and personal care now part of newly created neighbourhood teams. These teams will deliver personalised support across a variety of settings, including homes, schools, care homes, and community centres, ensuring that care is both accessible and tailored to individual needs.
Dr Amanda Webb, Chief Medical Officer at the Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board (BSW ICB), hailed the development as a “pivotal moment for the NHS.” She explained that the next two years will see a transformation in the way care is delivered, with a focus on early intervention and prevention to help residents stay healthier for longer.
“There’s been a real shift towards community-based care,” Dr Webb said. “The aim is to provide more personalised services, tailored to what matters most to patients. This change will help relieve pressure on hospitals and GP surgeries by treating health issues closer to home.”
Patrick Birchall, Managing Director of HCRG Care Group, echoed these sentiments, noting that the new approach will build on existing partnerships to ensure that people receive the best possible care, whether they are managing chronic conditions or preventing ill health altogether.
The transition, set to take place from 1 April 2025, will also see the creation of a “single front door” for people seeking care and support. Whether in person, online, or over the phone, this new access point will streamline services, ensuring residents receive the help they need quickly and efficiently.
The initiative will be supported by an annual £144 million in funding, with £7 million per year earmarked for building community capacity by partnering with local voluntary, community, and social enterprise (VCSE) providers. Outdated or unsuitable buildings will be refurbished or replaced, ensuring care facilities match the modern approach being introduced.
HCRG Care Group, which has been delivering NHS services in the region since 2006, brings a wealth of experience to the table. The Care Quality Commission has previously rated many of its services as “good” or “outstanding,” further establishing their credibility in transforming local care.
Cllr Richard Clewer, Chair of the BSW Integrated Care Partnership, praised the collaborative approach being taken. “True partnership working adds real value to patient care, and HCRG Care Group has a proven track record in this regard,” he said. “This is a significant step forward for our communities, and I’m confident we will see real benefits over the coming years.”
